Certificate of School  Activities


유의사항


   1. 본   학교생활증명서는         해외고교      출신자    중  학교생활기록부가          없는   경우    학교생활기록부에         해당하는      내용에    대하여     제출할
      수   있습니다.
      Applicants    who   studied   abroad   can   submit   this  certificate  of  school   activities which   consists   of
      extracurricular  activities if their high school  does  not  provide  School   Life Records.
   2.  본  학교생활증명서는          출신   고교에서      발행하는      양식이    없는    경우   활용할     수  있으며,     출신   고교의     양식(Resume      등)이
       별도로    있는   경우    출신   고교   양식으로      제출할    수  있습니다.
       Students  can  use  other  certificate of school  activities templates   if their school provides   an  official form
       of certificate with the  school  seal  or signature.
   3. 본   학교생활증명서는         반드시     학교장    서명   또는    학교   직인을    받아서     제출해야     합니다.
      Applicants   must   obtain  an  official signature  of the  headmaster    or  an  official seal from  the  institution
      they  attended.
   4. 2개   이상의    고교에     재학한    경우   해당    고교에서     각각   확인받아서       제출하시면      됩니다.
      If  applicant   attended   more    than  one   high   school,   he/she   needs   to   submit   separate   documents
      confirmed   by  each  institution.
   5.  본  학교생활증명서는          선택서류가       아니라    성적증명서의        추가서류로       간주합니다.      또한    활동내용에      따라    양식을     수정하여
       제출할    수  있으며,    페이지     수의   제한은    없습니다.
       This  certificate is not  an   additional  document,    but  a  sort of  transcript.  Applicants  may   modify   the
       template  according   to the  contents   of activities and  there is no  limit to the  number    of pages  that  you
       can  add  to the  certificate.
   6. 본   학교생활증명서는         내용에     대한   증빙서류는       별도로    제출할    필요가     없습니다.
      No   references  or evidential  documents    are  required  in addition  to the  certificate.
   7. 허위사실      기재   판명   시  불합격,    합격취소      또는   입학취소     등의    불이익이     있을   수  있습니다.
       If any false  information  is found,  it will result in cancellation of  admission.
